Case management is when a Bienestar staff member works closely with you and connects you to services to help you achieve your best state of health and well-being. Our case managers take time to understand you and all of your needs as a person. They will talk with you about all different areas of your life and evaluate your strengths, weaknesses, needs and resources. Our case managers will work with you to create a plan that improves your quality of life.

At Bienestar, our case managers do not just serve as someone to connect you with other community-based agencies. They will also advocate for you and address any challenges that may impact treatment and/or access to other medical or mental health services.

We can help you apply for the following services and resources:

  • Benefits
  • Counseling
  • Food
  • Medical Care
  • Shelter/Housing
  • Transportation
  • Other services you may need

Our case managers can also provide healthcare education to individuals, partners and families. They can talk to you about the risks and benefits of different medical treatments, managing an ongoing disease, issues related to medication and treatment, and how to prevent HIV, substance abuse and other infections or disorders.
